The last of Desmond's Memories -- being captured by Abstergo -- and as it is the final memory it is fitting that it verges on the real world in where we go... Sort of. As the memory begins walk forward and into the lift, and then create a set of blocks to rise up the lift shaft on the data stream, and when you get to the top go out and you find yourself in a datacube room. Follow the path until you reach a line of cubes that scattered ahead of you making a path and, as you cross it, look to the left at the first split and you will see the first of the Multi-Player Emblems that we need to grab here. Grab it and continue along the path.
Desmond's Fifth Memory: Kidnapped!
At the next intersection you will need to move forward and from side-to-side avoiding the grids to get to the far end where there is a glowing hand. Activating it shows you the route to the next glowing hand, which is littered with moving data cubes so avoid those and make your way along the indicated path, moving around the data cubes to the far side, where the next leg has a grid that moves along it.
The trick to getting past this is wait until it comes to you and heads back then follow it a short way before ducking on to the ledge on the right side, then wait for the grid to come back and pass you before you quickly head back into the path and to the end before it can catch up!

Now go around the corner and into the lift to activate the glowing hand like before -- create some blocks and ride them up to the next level and exit into the hallway there. Turn right and then around the left corner, follow the path building your blocks as needed and you turn the corner, making your way to the end and the open door on the right.
If you played the previous games this room will be intimately familiar to you -- it is the prison in which you were held throughout the game! Never expected to see this room again! The portal out is right ahead of you in the center of the room but, before you take that there is the matter of the 10th and final Multi-Player Emblem, which is up in the corner near the ceiling on the left side in the alcove with the windows.
Once you have that take the portal to unlock the Achievement "Are You Desmond Miles?" (20 GS) and there you are mates, you have completed all of the Desmond Sequences, all of the rest of the optional stuff -- all that is left is to complete the story! Well done indeed!
